What is a Life Certificate (Pensioner)?
A life certificate (jeevan pramaan patra) is the annual confirmation a pensioner gives to their pension-disbursing bank or treasury that they are alive, so that pension payments continue. For central government pensioners it is usually submitted in November, and pensioners aged 80 and over can submit from October.
It can be submitted in person at the bank, digitally through Jeevan Pramaan (biometric or face authentication), or as a signed physical certificate. This template gives a physical certificate for pensioners who submit on paper.

How to make it legally valid
- Signed by the pensioner, with the PPO and account numbers exactly as in bank records.
- Attested by an authorised officer where the pensioner does not appear in person, if the bank requires it.
- Submitted within the period your pension authority specifies, to avoid suspension of pension.
