Stamp paper required — non-judicial stamp paper of the value prescribed by your state. You'll receive the draft; e-stamping is done separately per your state's rules.

What is a Passport Name/Address Affidavit?

Passport applications are checked hard against supporting documents, and a mismatch stops the file. A name spelt one way on the school certificate and another on the PAN card, an address that moved between the Aadhaar and the bank statement — the Passport Office asks you to explain it on oath before it proceeds.

The important thing to know first: the Ministry of External Affairs publishes its own annexures for passport applications, and for many situations it requires ITS format, not a general affidavit. Check the current annexure list on the Passport Seva portal before you pay for stamp paper. Use this affidavit where no annexure covers your situation, or where the office has asked for a general affidavit.

When to use it

  • Your name is spelt differently across the documents you are submitting with the passport application.
  • Your address differs between your Aadhaar, bank records and the address you are applying from.
  • An earlier document carries a maiden name or a pre-correction spelling.
  • The Passport Seva Kendra has asked you for an affidavit explaining a discrepancy.
  • You are applying for re-issue and your particulars have changed since the last passport.

How it works

  1. Check the Passport Seva annexures first

The MEA publishes annexures for most passport situations — name change after marriage, single parent, minor’s consent, and others. Where an annexure covers your case, use it: a general affidavit will be refused in its place.

  2. Quote both versions exactly

Copy the old and the new spelling or address exactly as they appear on the documents, character for character. The whole point is to link the two, and an approximate quotation defeats it.

  3. List the documents affected

Naming which documents carry the old version tells the verifying officer exactly what they are looking at.

  4. Preview and download

Check the draft, pay ₹50, download the PDF.

  5. Print on stamp paper and notarise

Print on non-judicial stamp paper of your state’s value and swear it before a Notary Public, carrying every document mentioned in it.

How to make it legally valid

  • Non-judicial stamp paper of the value prescribed by your state.
  • Sworn in the physical presence of a Notary Public or First Class Magistrate.
  • Notary’s signature, seal and register entry.
  • Where the MEA prescribes an annexure for your situation, use the annexure — it overrides this.
  • For an actual change of name, rather than reconciling a discrepancy, the Passport Office asks for newspaper publication and, for reasons other than marriage or divorce, Gazette notification.

Stamp paper. Print the affidavit on non-judicial stamp paper. The value is fixed by your state, not by us — commonly Rs. 10 in Delhi and Uttar Pradesh, Rs. 20 in Karnataka and Tamil Nadu, Rs. 50 in Telangana and Andhra Pradesh, and Rs. 100 in Maharashtra and Gujarat. Ask the stamp vendor or your notary for the current value in your state: using a lower denomination than required is the most common reason an affidavit is handed back.

Common mistakes to avoid

  • Using a general affidavit where the MEA prescribes an annexure. That is the commonest reason these are rejected at the counter.
  • Confusing a discrepancy with a name change. A spelling variation is reconciled by affidavit; adopting a new name needs the full newspaper and Gazette process.
  • Quoting the old spelling approximately instead of exactly as printed.
  • Not carrying the documents named in the affidavit to the appointment.
  • Leaving it to the day of the appointment — stamp paper and a notary both take time.

Frequently asked questions

Does the Passport Office accept a general affidavit?

Sometimes, but the Ministry of External Affairs publishes its own annexures for most passport situations, and where an annexure applies it must be used. Check the current annexure list on the Passport Seva portal before preparing anything else.

What is the difference between a discrepancy and a name change?

A discrepancy is a variation — a spelling difference, an initial expanded, a maiden name on an old certificate — where you are not adopting a new name. A name change is adopting a genuinely different name, which needs a name change affidavit, newspaper publication and, for reasons other than marriage or divorce, Gazette notification.

I changed my name after marriage. What does the passport office need?

For a name change on marriage, the MEA generally accepts the marriage certificate along with its prescribed annexure, without requiring Gazette notification. For a change after divorce, the decree is used. Check the current requirement on the Passport Seva portal.

Do I need Gazette notification for a passport name change?

For a name change made for any reason other than marriage or divorce, yes — the Passport Office asks for the Gazette notification along with newspaper clippings.

My address differs from my Aadhaar. What do I do?

The cleanest route is to update the Aadhaar address first, since the Passport Office verifies against it. Where that is not possible in time, this affidavit explains the difference; carry the documents evidencing the current address.

Does it need to be notarised?

Yes. The Passport Office will not accept an unnotarised declaration.

What stamp paper value applies?

The ordinary affidavit value in your state, commonly Rs. 10 to Rs. 100.

Will this help with police verification?

It helps the verifying officer understand a discrepancy, but police verification is a separate process based on your residence and record. Keep your address documents ready for it.

Can I submit this for a minor’s passport?

A parent or guardian swears it on the minor’s behalf. Note that minors’ passport applications have their own annexures, including for consent where one parent is unavailable — check the portal.

How recent must the affidavit be?

Execute it close to your appointment date. Passport Seva Kendras generally expect a recent affidavit.
